Explain ‘outcome based practice’

Google says “Outcome based practice is a process that has a beneficial impact on the people we support’s life. It can be an action taken or the service that we deliver. Outcome based approaches place the needs of the person at the centre of the service they receive. Another way to describe it would be to say that the output is the effort made and the outcome is the effect on the person. Outcome based practice is of growing importance in any care service. It ensures that the service we deliver is person centred to the persons needs and enables staff to deliver the best care possible.” !!
